Welcome to THINK BREAKFAST! 2008-2009
We are now accepting online applications from NYS teachers (grades pre-K to 5) who would like to receive Think Breakfast! classroom materials to use during the Winter 2008-2009 school year. You can register as an individual, or your school can submit a group application form, after designating an individual to coordinate orders at your school. (Please try to avoid submitting duplicate orders.)
This is a classroom-based initiative and is not suitable for physical education, art or music classes. It is designed around three-weeks of classroom activities and instruction.
> Individual Teachers K-5
We prefer that each teacher complete and submit an individual order form for his/her class. The Think Breakfast! package of materials will then be custom-collated and packaged to match the number of students in the teacher's class, as noted on his/her order form. To insure no duplication of efforts, teachers please check first to see if your school is submitting orders as a group. Do not submit an individual order form if your school is submitting a Group Order Form.

> Multiple Teacher Orders for One School
If your school designates an individual (for example the school nurse, health teacher, or administrative assistant) to coordinate Think Breakfast! orders at your school, this individual may submit orders for multiple teachers by completing the GROUP ORDER FORM (click on link below). The coordinator will receive all of the ordered materials, and will be responsible for distributing the materials to teachers who placed orders with the coordinator. Health Instructors: if you are not the designated Think Breakfast! coordinator, please check with the other teachers at your school before ordering, to alleviate duplicate orders/efforts.
NOTE: if you are registering more teachers than the application allows, please repeat the process as many times as needed to accommodate all your teacher's names.
